  • Grade 12 Report Card

    A scanned copy of your latest report card or Form 138 from your previous school.

  • PSA Birth Certificate

    A scanned copy of your PSA Birth Certificate.

  • 2x2 Photo

    Recent 2x2 ID photo with white background and your name printed at the bottom.

  • PSA Marriage Certificate

    A scanned copy of PSA Marriage Certificate (for married female applicants only).
